If scalp hair cannot be obtained, a similar volume of body hair can be used. When mentioned, head hair refers exclusively to scalp hair. Body hair encompasses all other types (e.g., facial, axillary).

Process Overview

There are four pivotal steps in the laboratory assessment of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first stage where a sample is integrated into a lab’s system. This involves confirming that the sample was properly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and entering additional data not covered by an electronic chain of custody.

Screening is a preliminary examination for drug abuse indicators. While it’s a cost-efficient method for eliminating drug use in most samples, a positive result requires confirmation to hold up in legal contexts. Presumptively positive samples from Screening necessitate a follow-up confirmation.

If the sample tests presumptively positive during Screening, additional hair is taken from the original specimen for Extraction. In this phase, drugs are extracted from hair at much lower concentrations compared to other methods (such as urine or oral fluid), which makes hair screening one of the more challenging methodologies.

Confirmation for positive screening results is handled using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples deemed presumptively positive undergo washing prior to confirmation if required. The complete lab process from Accessioning through Confirmation is examined under the standards of both the CAP (College of American Pathologists) Hair designation and accreditation according to ISO / IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
