For EtG, add-ons, and tests exceeding 10 panels, a 150mg specimen is advised. It is suggested to weigh the specimen with a jeweler’s scale. If scalp hair isn't accessible, a similar quantity of body hair is acceptable. In this context, head hair refers strictly to scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The drug testing process in the lab is divided into four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the stage where a sample is initially processed into a laboratory’s system. This step involves confirming that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry missing from an electronic chain of custody system.

Screening serves as a preliminary check for substances of abuse. Although Screening offers a cost-effective way to dismiss drug use in most samples, a positive result must be verified to be considered valid in legal settings. Samples with presumptively positive results in Screening require secondary confirmation.

Should a sample test positive in the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. During Extraction, drugs are extracted from hair at considerably lower concentrations compared to other techniques (like urine or oral fluid), which is why hair drug testing is the most complex methodology to execute.

Confirmation of any screened positive result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation if needed. The entire lab process from Accessioning to Confirmation is overseen under the CAP (College of American Pathologists) Hair designation and adherence to ISO/IEC 17025 standards.
