For EtG, extras, and/or panels exceeding 10, a sample of 150mg is suggested. Using a jeweler’s scale is advisable for weighting the specimen. If scalp hair isn't available, a comparable amount of body hair can be sampled. When talking about head hair, it specifically means scalp hair, while body hair encompasses other types (facial, underarm, etc.).

Process Overview

The laboratory procedure for processing a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial sample handling within a lab’s system. This involves confirming the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent in an electronic chain of custody system.

Screening entails an initial, rapid check for drug abuse. Screening is economical for eliminating drug presence in most samples, but a positive result requires secondary confirmation for courtroom use, since presumptive positive results in Screening need verification.

Should a sample yield a presumptive positive during Screening, additional hair is extracted from the original sample for Extraction. This stage involves extracting drugs from hair at far lower concentrations than alternative methods (e.g., urine, oral fluid), thus making hair drug testing the most challenging method to execute.

All positive screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, presumptive positives are washed as needed. The entire lab process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair qualification and accredited to ISO / IEC 17025 standards.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
