What it is, How it works

Hair testing has emerged as a significant tool for detecting drug and alcohol consumption. By integrating biomarkers within the strands of hair as it grows, hair offers a comprehensive history of alcohol and drug use. When trimmed close to the scalp, hair provides a detection window that can last up to roughly three months for alcohol as well as other drugs. The process of collecting hair is straightforward, less prone to tampering, and facilitates easy shipment.

A sample measuring 1.5 inches comprising roughly 200 hair strands (similar to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample amount for both screening and confirmation. For tests that include EtG, additional elements, and/or exceed 10 panels, it is advised to collect 150mg of the sample. Using a jeweler’s scale is the recommended method for weighing the specimen. In cases where scalp hair isn’t available, an equivalent quantity of body hair can be used. References to head hair specifically mean scalp hair, whereas body hair includes any other types such as facial and axillary hair.

Process Overview

The laboratory processes a drug test result through four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails integrating a sample initially into a laboratory’s database. This process involves checking that the sample is correctly sealed and shipped, assigning a LAN (Laboratory Accessioning Number) randomly, and completing any missing data entry not covered by an electronic system for chain of custody.

Screening functions as an initial rapid test to detect drugs of abuse. Although Screening provides a budget-friendly method to screen out the majority of samples with no drug usage, samples that yield positive screens must be validated for courtroom acceptance. Samples showing presumptive positives in Screening necessitate a follow-up confirmation.

If a presumptive positive result occurs during the Screening phase, additional hair is extracted from the original specimen for preparation in the Extraction phase. Here, drug substances are retrieved from hair at concentrations substantially lower compared to other methods, such as urine or oral fluid, contributing to the challenge of hair drug screening.

GC/MS, GC/MS/MS, or LC/MS/MS techniques are employed for the Confirmation of any positive Screening results. All presumptively positive samples undergo washing as required before confirmation. The comprehensive laboratory workflow from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair standards alongside the ISO / IEC 17025 accreditation requirements.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
