A hair sample measuring 1.5 inches, consisting of approximately 200 strands (similar to the thickness of a #2 pencil), provides 100mg of hair, considered optimal for both screening and verification. For EtG, add-ons, and/or tests exceeding 10 panels, a sample of 150mg is suggested. Utilizing a jeweler's scale is recommended for measuring the sample weight. In cases where scalp hair isn't available, an equivalent volume of body hair can be used. When mentioning head hair, it specifically means scalp hair. Body hair refers to any other types of hair (such as facial, underarm, etc.).

Process Overview

During laboratory processing of a drug test, the four critical stages are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial procedure of entering a specimen into a lab's system. This step ensures the sample was properly sealed and delivered, assigns it a random LAN (Laboratory Accessioning Number), and completes any necessary data entry absent from an electronic chain of custody system.

Screening offers an initial evaluation for drugs of abuse. Despite Screening being an economical approach to exclude drug use in most samples, a positive result necessitates confirmation to be legally recognized. Samples that are initially positive in Screening require additional confirmation.

Should a specimen yield a presumptively positive Screening result, additional hair is extracted from the original sample for the Extraction process. At this phase, drugs are withdrawn at much lower concentrations compared to other methods (e.g., urine or oral fluid), which contributes to the complexity of hair drug analysis.

Confirmation for any positive Screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Samples that initially test positive undergo washing before Confirmation if necessary. The entire lab procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair endorsement and accreditation per ISO / IEC 17025 criteria.
