A 1.5-inch segment of approximately 200 hair strands (around the diameter of a #2 pencil) nearest the scalp yields 100mg of hair, which is optimal for both screening and confirmation. For EtG, additional tests, or panels exceeding 10, a specimen of 150mg is suggested. Using a jeweler’s scale for weighing the specimen is recommended. If scalp hair isn't available, a similar mass of body hair can be collected. By head hair, we specifically mean scalp hair, while body hair includes other types (facial, axillary, etc.).

Process Overview

The laboratory drug testing process is divided into four major phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the introductory processing of a sample into the lab's system. Tasks in this phase include ensuring the sample was securely sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and completing any remaining data entry that isn't managed by an electronic chain of custody.

Screening consists of an initial swift assessment for drug presence. While Screening is an economical way to exclude drug usage in most samples, a positive outcome needs confirmation for legal acceptability. Any samples that test presumptively positive during Screening must undergo additional confirmation.

When a sample is presumptively positive in the Screening step, additional hair is extracted from the original sample for the Extraction process. Here, drugs are isolated from hair in much smaller concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening one of the most challenging techniques.

Confirmation of any positive results from the screening is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as necessary. The comprehensive lab workflow from Accessioning to Confirmation is assessed both under the CAP (College of American Pathologists) Hair designation and the accreditation standards of ISO / IEC 17025.
