What it is, How it works

Hair offers a prolonged record of alcohol and drug intake by capturing biomarkers within the fibers of the developing hair strand. When gathered near to the scalp, hair can reveal up to a roughly 3-month timeframe for detecting alcohol and drugs. Collecting hair is straightforward, challenging to tamper with, and convenient to send.

A sample measuring 1.5 inches consisting of roughly 200 hair strands (equivalent to a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for analysis and validation. For EtG, additional tests, and/or tests exceeding 10 panels, a 150mg specimen is advised. We suggest assessing the specimen on a jeweler’s scale. If scalp hair is not available, a comparable quantity of body hair can be used. When discussing head hair, this pertains only to scalp hair. Body hair encompasses other hair types (facial, axillary, etc.).

Process Overview

The four key steps in laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the primary processing of a sample into a lab’s system. This involves confirming that the sample was properly sealed and sent, allocating a random LAN (Laboratory Accessioning Number), and completing any additional data input not supplied by an electronic chain of custody system.

Screening is a swift initial check for drugs of abuse. While Screening is a cost-effective method to exclude drug usage on most samples, a positive Screen must be verified to be acceptable in court. Samples that screen positive on a preliminary basis need an additional confirmation.

If a sample screens positive during the Screening stage, more hair is taken from the initial sample and prepared for Extraction. In this stage, drug extraction occurs at a significantly lower concentration than in other methods (e.g., urine or oral fluid), making hair drug screening a more challenging method to perform.

Confirmation for any positive screening result is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleaned before confirmation as required. The whole lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation standards.
