What it is, How it works

Hair testing has emerged as a potent instrument for identifying drug and alcohol consumption. Hair provides an extended history of alcohol and other drugs by embedding biomarkers in the strands of growing hair. When collected near the scalp, hair can deliver up to a 3-month window of detection for alcohol and other substances. Hair is easy to gather, relatively hard to tamper with, and simple to transport.

A 1.5-inch segment of approximately 200 hair strands (similar in size to a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the quintessential sample for testing and verification. For EtG, additional tests, and/or those exceeding 10 panels, a 150mg sample is suggested. We advocate weighing the sample using a jeweler’s scale. Should scalp hair be unobtainable, an equivalent volume of body hair may be collected. In reference to head hair, we mean scalp hair strictly. Body hair encompasses all other hair variants (facial, axillary, etc.).

Process Overview

The four fundamental steps in laboratory drug test processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial integration of a sample into a laboratory system. This process ensures the sample was sealed and shipped adequately, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary data entry omitted by an electronic chain of custody mechanism.

Screening entails an initial rapid verification for substances of abuse. Although Screening is a cost-efficient strategy to exclude drug usage for most samples, a positive screen must be validated to be legally valid. Samples that test potentially positive in Screening require a follow-up confirmation.

Should a sample appear potentially positive in the Screening phase, additional hair is extracted from the primary sample and set up for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than in alternative procedures (e.g., urine or oral fluid), making hair drug screening one of the most challenging methodologies to execute.

Confirmation of a positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All tentatively positive samples are cleansed prior to confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation undergoes review according to both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
