What it is, How it works

When gathered near the head, hair can reveal up to a three-month span of alcohol and drug usage. Hair is easy to gather, somewhat resistant to tampering, and straightforward to transport.

A 1.5-inch segment of roughly 200 hair strands (approximately the thickness of a #2 pencil) nearest to the head yields 100mg of hair, the optimal quantity for testing and validation. For EtG, additional tests, and/or panels exceeding ten, it's advised to use 150mg of the sample. We advise using a jeweler’s scale to weigh the sample. If head hair is missing, a similar amount of body hair can be collected. When referencing head hair, we mean scalp hair exclusively. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory process of generating a drug test result comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary phase where a sample is entered into the lab system. This step includes confirming the sample was securely sealed and transported, assigning a random Laboratory Accessioning Number (LAN), and performing any further data entry not handled by an electronic custody system.

Screening entails a preliminary evaluation for substances of abuse. While Screening offers a cost-effective solution to dismiss drug use in most cases, a positive find requires corroboration to be legally valid. Any samples with a presumptive positive result in Screening necessitate follow-up confirmation.

Should a sample display a presumptive positive during Screening, additional hair is extracted from the initial sample for the Extraction phase. Here, drugs are separated from hair at significantly lower concentrations than with other methods (e.g., urine or saliva), thus making hair drug screening one of the most challenging methodologies to execute.

To confirm any positive Screening outcomes, techniques such as GC/MS, GC/MS/MS, or LC/MS/MS are employed. All samples with presumptive positive results are cleansed before confirmation if needed. The entire lab sequence from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
