A sample of 1.5 inches consisting of about 200 hair strands (comparable to a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the optimal sample quantity for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, a 150mg sample is advised. It's advisable to measure the specimen with a jeweler’s scale. If scalp hair cannot be obtained, an equivalent measure of body hair may be used. When referencing head hair, it pertains exclusively to scalp hair. Body hair encompasses all other types of hair (such as facial, axillary, etc.).

Process Overview

The procedure for laboratory processing of drug test specimens encompasses Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample into a lab's system, which includes ensuring the sample is adequately sealed and transported, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data input not facilitated by an electronic chain of custody system.

Screening involves promptly checking for drugs of misuse. Although Screening efficiently rules out drug consumption in most samples, a positive result requires confirmation to be legally valid. Samples that return a presumptive positive in Screening need a subsequent confirmation.

If Screening returns a presumptive positive, additional hair is extracted from the initial sample for Extraction. In this phase, drugs are isolated from hair at substantially lower concentrations compared to other techniques (such as urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation, as needed. The comprehensive laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
