What it is, How it works

Hair analysis is now recognized as an effective method for identifying drug and alcohol consumption. Hair retains a long-term record of alcohol and other drugs by ensnaring biomarkers within the fibers of the growing strand. When gathered near the scalp, hair can offer an approximate 3-month detection period for alcohol and other substances. Hair is straightforward to obtain, challenging to tamper with, and simple to transport.

A 1.5-inch segment of about 200 hair strands (resembling the size of a #2 pencil) taken closest to the scalp yields 100mg of hair, which is considered the optimal sample for both screening and confirmation procedures. For EtG, additional panels, and/or tests exceeding 10-panel screenings, 150mg of the sample is suggested. It is advisable to weigh the sample using a jeweler’s scale. In the absence of scalp hair, an equivalent amount of body hair may be collected. Our reference to head hair specifically relates to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial processing of a sample within a lab's system. This process ensures the sample was correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any outstanding data entry not provided through an electronic chain of custody.

Screening is a preliminary, cost-effective check for drugs of abuse. Although Screening is effective for ruling out drug use in most samples, any positive results require confirmation to be valid for legal proceedings. Samples that presumptively test positive in Screening mandate a secondary confirmation.

For samples that test presumptively positive during Screening, additional hair is taken from the original sample and readied for Extraction. At this stage, drugs are extracted from hair at significantly lower concentrations than through other methods (e.g., urine or oral fluid), contributing to the complexity of performing hair drug screening.

Confirmation of any positive screening findings is executed through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that presumptively test positive are cleansed prior to confirmation if necessary. The complete laboratory process, from Accessioning to Confirmation, is reviewed under the CAP (College of American Pathologists) Hair designation and in accordance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
