What it is, How it works

When obtained near the scalp, hair can reveal alcohol and drug usage within an approximately three-month period. Hair collection is straightforward, hard to tamper with, and easy to transport.

A 1.5-inch sample consisting of around 200 strands (roughly the size of a #2 pencil) from near the scalp provides 100mg, which is optimal for screening and confirmation. For EtG, additional assessments, and/or testing beyond 10 panels, it is advisable to have 150mg of the sample. It's advisable to weigh the sample using a jeweler’s scale. If scalp hair is not available, a similar amount of body hair can be used. When we mention head hair, it indicates scalp hair, excluding all other hair types (facial, axillary, etc.) referred to as body hair.

Process Overview

The lab processing of a drug test outcome entails four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is about incorporating a sample into the laboratory’s system. It includes affirming the sample's proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing additional data entry if the electronic chain of custody system lacks it.

Screening comprises an initial swift evaluation for abusive drugs. Though Screening is cost-efficient to exclude drug usage in most samples, a positive result requires confirmation for court acceptance. Samples showing presumptive positives in Screening need further confirmation steps.

If Screening suggests a presumptive positive result, extra hair is retrieved from the sample and made ready for Extraction. Here, drugs are extracted from hair at a significantly lower concentration compared to other techniques (e.g., urine or oral fluid), making hair drug screening particularly challenging.

Confirmation of any positive Screening result employs GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing as needed before confirmation. The complete lab procedure from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and follows ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
