Should scalp hair be unavailable, an equal measure of body hair can be used. When mentioning head hair, this strictly means scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four foundational steps in the lab assessment of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the inaugural handling of a sample within a lab’s system, encompassing checking that the sample was adequately sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and performing any additional data input absent from an electronic chain of custody system.

Screening entails an initial, rapid evaluation for drug misuse indication. While Screening is cost-efficient for ruling out drug consumption in most samples, confirmed validation is necessary for court acceptance. Samples initially flagged as positive during Screening necessitate further confirmation.

In cases of suspected positive results during the Screening phase, additional hair is extracted from the primary sample and prepared for Extraction. During this phase, drugs are isolated from hair at significantly lower concentrations compared to other methodologies (e.g., urine or oral fluid), thus making hair drug screening the most challenging to conduct.

Confirming any positive Screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicated as positive are cleansed before confirmation when required. The comprehensive laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and certification to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
