What it is, How it works

When collected near the scalp, hair can offer a detection window of up to roughly 3 months for alcohol and other substances. Hair collection is straightforward, relatively difficult to tamper with, and easy to transport.

A 1.5-inch sample of around 200 hair strands (approximately the size of a #2 pencil) closest to the scalp will yield 100mg of hair, the optimal sample for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the specimen is advised. We suggest weighing the specimen using a jeweler’s scale. If scalp hair cannot be obtained, an equivalent quantity of body hair may be used. When referring to head hair, we mean scalp hair only. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The primary steps in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial entry of a sample into a laboratory’s system. This step checks that the sample was properly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and completes any other required data entry not covered by an electronic chain of custody system.

Screening serves as the initial rapid assessment for drugs of abuse. Although Screening is a cost-effective method to discount drug usage in most samples, a confirmed positive screen is necessary for court acceptance. Samples showing presume-ably positive results in Screening need secondary confirmation.

If a sample is notably positive in the Screening stage, additional hair is removed from the original specimen and prepared for Extraction. At this stage, drugs are extracted from hair at much lower concentrations than in other methodologies (e.g., urine or oral fluid), making hair drug screening one of the more challenging methodologies to execute.

Confirmation of any positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All potentially positive samples are washed as needed before confirmation. The entire laboratory process, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
