A sample measuring 1.5 inches, comprising about 200 hair strands (roughly the diameter of a #2 pencil) taken closest to the scalp, will yield 100mg of hair, which is the ideal sample size for both screening and confirmation. For tests such as EtG, add-ons, or those exceeding 10 panels, a 150mg sample is advisable. Using a jeweler’s scale to weigh the specimen is recommended. If scalp hair isn't available, an equivalent amount of body hair can be collected. Here, head hair specifies scalp hair, while body hair refers to other hair types (like facial or axillary).

Process Overview

The laboratory processing of a drug test comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample enters a laboratory’s system. This step includes checking that the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry not covered by an electronic chain of custody system.

Screening is a preliminary check for drugs of abuse. Though it's a cost-effective means to identify drug usage in most samples, a positive screen needs further confirmation to be court-admissible. Samples testing presumptively positive require a secondary confirmation.

In cases where a sample tests positive in Screening, more hair is extracted from the initial sample and prepared for Extraction. This phase involves removing drugs from the hair at concentrations much lower than other methods (such as urine or oral fluid), pointing to why hair drug screening is more challenging to conduct.

Confirmation of a positive screening result utilizes GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples undergo washing as necessary before confirmation. The entire laboratory journey from Accessioning to Confirmation undergoes scrutiny under both the CAP (College of American Pathologists) Hair designation and the accreditation adhering to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
