When taken near the scalp, hair can offer close to a 3-month window for alcohol and drug detection. Hair is easy to collect, somewhat resistant to tampering, and straightforward to send.

A 1.5-inch segment consisting of around 200 hair strands near the scalp provides 100mg of hair, which is ideal for screening and confirmation tests. For EtG, additional tests, or those over 10 panels, 150mg is advisable. It's suggested to use a jeweler’s scale for weighing. If scalp hair isn't available, a similar quantity of body hair can be used. In terms of head hair, we refer specifically to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first stage of processing a sample into a lab’s system. This step ensures proper sealing and shipment of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any further data entry not done by an electronic chain of custody system.

Screening offers an initial rapid assessment for drug abuse. It's an affordable method to exclude drug use for most samples, yet a positive screening must be verified for court acceptability. Presumptive positive samples in Screening need extra confirmation.

When a sample tests presumptively positive initially, additional hair from the original specimen is used for Extraction. Here, drugs are retrieved from hair at lower concentrations than other methods (such as urine or oral fluid), presenting a challenge in hair drug testing.

Verification of positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The lab process from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
