A hair sample measuring 1.5 inches and containing roughly 200 strands (equivalent to the girth of a #2 pencil) from close to the scalp yields 100mg, which is the perfect quantity for both screening and confirmation. For EtG, additional testing components, and/or assessments exceeding 10 panels, a 150mg sample is suggested. It is advisable to use a jeweler’s scale for measuring the specimen. If scalp hair is absent, an equivalent quantity of body hair is acceptable. Specifically, when we mention head hair, we are indicating scalp hair exclusively. Body hair encompasses all other hair varieties (such as facial and axillary).

Process Overview

The laboratory processing of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a specimen into a laboratory's system. This step includes ensuring the specimen was correctly sealed and transported, issuing a random LAN (Laboratory Accessioning Number), and completing any missing data entry not furnished by an automatic chain of custody framework.

Screening entails a preliminary rapid assessment for narcotics. While it is an economic measure to dismiss substance use for most specimens, a positive screen demands confirmation for legal admissibility. Samples that exhibit preliminary positivity necessitate subsequent confirmation.

Upon registering a presumptive positive at the Screening phase, additional hair is drawn from the original sample for the Extraction phase. Drugs are isolated from hair at lower concentrations than other techniques (like urine or oral fluid), which accounts for hair drug analysis being the most complex technique to execute.

Any positive result detected during screening undergoes validation through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples that test presumptively positive are cleaned as necessary. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
