What it is, How it works

When obtained near the scalp, hair can offer up to a three-month detection period for alcohol and drugs. It is easy to gather, challenging to tamper with, and convenient to transport.

A 1.5-inch section consisting of about 200 hair strands (roughly the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal amount for both screening and validation. For EtG, additional tests, and evaluations exceeding 10 panels, a sample of 150mg is advised. Ensuring the specimen's weight on a jeweler’s scale is recommended. If scalp hair is not accessible, an equivalent quantity of body hair can be used. By head hair, we specifically mean scalp hair. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the preliminary processing of a sample in a laboratory’s system. This step involves confirming that the sample was properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any other required data entry absent from an electronic chain of custody system.

Screening includes a preliminary rapid assessment for drugs of abuse. While Screening offers a cost-efficient means to eliminate drug use in most samples, a positive screening result requires confirmation to be valid for legal proceedings. All presumptive positive samples during Screening necessitate a secondary confirmation.

Should a sample be presumptively positive during the Screening phase, additional hair is retrieved from the initial sample for preparation in the Extraction phase. Here, drugs are extracted from hair at a significantly lower concentration compared to alternative methodologies (such as urine or oral fluid), highlighting the complexity of hair drug screening.

Positively screened samples are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that are presumptive positive are cleansed before confirmation if needed. The entire laboratory procedure, from Accessioning to Confirmation, is assessed under the CAP (College of American Pathologists) Hair protocol and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
