A sample measuring 1.5 inches of roughly 200 hair strands (about the thickness of a standard #2 pencil) closest to the scalp provides 100mg of hair, which is suitable for both screening and confirmation. For EtG, additional tests, and/or panels over 10, a 150mg specimen is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is not available, a comparable quantity of body hair can be taken. When referring to head hair, it pertains solely to scalp hair. Body hair encompasses all other hair kinds (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test result includes four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of incorporating a sample into the lab’s system, comprising checking that it is appropriately sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing further data entry absent from an electronic chain of custody system.

Screening performs a quick preliminary inspection for substance abuse. While it serves as a cost-effective method to rule out drug use for most samples, any positive screen requires confirmation to be legally admissible. Samples that test positive during Screening need a secondary confirmation.

If a Screening result is positive, additional hair is drawn from the original specimen for the Extraction phase. During this step, substances are extracted from hair at notably lower concentrations than in alternate methods (e.g., urine or oral fluid), which is why hair drug testing presents significant challenges.

Confirmation of positive screening results is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed prior to confirmation as necessary. The complete laboratory process, from Accessioning to Confirmation, is overseen in alignment with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
