What it is, How it works

Hair testing is now recognized as an effective method for detecting drug and alcohol consumption. When taken near the scalp, hair offers a detection period of up to about 3 months for alcohol and drugs. Hair is effortless to collect, hard to tamper with, and straightforward to transport.

A sample measuring 1.5 inches with approximately 200 hair strands (akin to the diameter of a #2 pencil) taken from the scalp will yield 100mg, the optimal amount for screening and confirmation. For EtG, additional testing, and/or panels exceeding 10, 150mg is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair is not available, an equivalent volume of body hair can be used. "Head hair" refers specifically to scalp hair, whereas "body hair" encompasses all other types (e.g., facial, axillary).

Process Overview

The drug test processing in the lab consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial step where a sample is processed into the lab’s system. It involves ensuring the sample is properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not provided by an electronic chain of custody system.

Screening is a quick initial check for drug misuse. While this is a cost-efficient method to exclude drug use in most samples, a positive finding must be confirmed to be valid in court. Samples presumptively positive in Screening require further confirmation.

If a sample is presumptively positive during Screening, additional hair is extracted from the original specimen and prepared for Extraction. In this stage, drugs are extracted from hair at notably lower concentrations compared to other methods (like urine or oral fluid), demonstrating why hair drug screening presents a methodologically challenging process.

Confirmation of any positive finding in screening is done through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The entire process from Accessioning to Confirmation is audited under the CAP (College of American Pathologists) Hair designation alongside the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
