For EtG, additional tests, and/or tests above 10 panels, 150mg of the sample is suggested. We advise weighing the sample using a jeweler's scale. If scalp hair is unavailable, a corresponding volume of body hair can be collected. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial introduction of a sample into a laboratory's system. This process includes ensuring that the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any remaining data entry absent in an electronic chain of custody setup.

Screening involves a preliminary rapid assessment for drugs of abuse. While it's cost-efficient to rule out drug use in most samples via Screening, a positive screen result necessitates confirmation for court admissibility. Any samples yielding presumptive positive results during Screening require further confirmation.

If a sample tests presumptively positive during Screening, more hair is taken from the original sample and readied for Extraction. During this phase, drugs are taken out from hair at a lower concentration than in other methods (e.g., urine or oral fluid), rendering hair drug screening the most complicated methodology.

Confirmation of any positive Screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive undergo pre-confirmation washing as necessary. The entire laboratory sequence from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
