For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the specimen is advisable. Using a jeweler's scale for measuring the specimen is suggested. If scalp hair isn't available, a comparable amount of body hair might be used. Head hair specifically refers to scalp hair only, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

There are four primary phases in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial entry of a sample into a lab's system. This involves confirming the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input not provided by an electronic chain of custody system.

Screening comprises an initial quick assessment for drug abuse. While Screening is an economical method to exclude drug use in most samples, a positive screen must be verified to be valid in court. All samples presumptively positive in Screening necessitate further confirmation.

If a sample turns out presumptively positive in the Screening phase, additional hair is taken from the original specimen and prepared for Extraction. During this stage, drugs are extracted from hair at significantly lower concentrations than with other methods (e.g., urine or oral fluid), thus making hair drug screening one of the most challenging methods to execute.

Confirmation of any positive screening outcome is conducted through GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
