If scalp hair is inaccessible, a similar amount of body hair may be used. Reference to head hair signifies scalp hair exclusively. Body hair refers to any other hair types (facial, axillary, etc.).

Process Overview

The primary steps involved in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial sample processing within a laboratory's system. This step involves ensuring the sample's proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry missing from an electronic chain of custody system.

Screening constitutes a preliminary rapid check for drugs of abuse. While Screening offers a cost-effective approach to dismiss drug usage in most samples, a positive screen must be confirmed for legal admissibility. Samples showing presumptive positivity in Screening necessitate a follow-up confirmation.

If a sample demonstrates presumptive positivity during the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. During this stage, drugs are extracted from hair at significantly lower concentrations than in alternative methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most challenging to conduct.

Confirmation of positive screening results is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples undergo washing before confirmation when required. The comprehensive lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
