A 1.5-inch portion of around 200 hair strands (akin to the size of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal amount for screening and verification. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg is suggested. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is not available, an equivalent quantity of body hair can be taken. References to head hair denote exclusively scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary stages in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial integration of a sample into a lab's system. This step verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any outstanding data entry not covered by an electronic chain of custody system.

Screening provides an initial evaluation for potential drug abuse. Any samples with presumptive positive results in Screening necessitate secondary confirmation.

Here, drugs are isolated from hair at significantly lower concentrations than through other methods (like urine or oral fluid), resulting in hair drug screening being among the most challenging methodologies.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed accordingly before confirmation. The whole laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the conformity to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
