What it is, How it works

Hair analysis is increasingly recognized as an effective method for detecting drug and alcohol consumption. Hair captures a comprehensive record of substance use by embedding chemical indicators within the growing strands. When the sample is taken near the scalp, it can offer an approximately three-month timeframe of detection for alcohol and drugs. Hair samples are easy to collect, challenging to tamper with, and simple to transport.

A segment of 1.5 inches, containing roughly 200 strands (comparable to a #2 pencil's thickness), right at the scalp, yields 100mg of hair, which is optimal for both screening and confirmation processes. For EtG, additional tests, and/or panels exceeding 10, a specimen of 150mg is advisable. We recommend using a jeweler's scale to measure the specimen. Should scalp hair be unavailable, a similar volume of body hair is acceptable. Here, head hair specifically alludes to scalp hair. In contrast, body hair encompasses all other types, such as facial or axillary hair.

Process Overview

The drug testing procedure in a lab involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab's system. This phase includes ensuring that the specimen was sealed and dispatched properly, allocating a unique LAN (Laboratory Accessioning Number), and completing any supplementary data input not facilitated by an electronic custody chain system.

Screening is a preliminary assessment for substances of abuse. While this is a cost-efficient method for excluding drug use in most specimens, any positive result needs verification to be valid in legal settings. Samples that yield tentative positives during Screening must undergo a secondary confirmation.

For specimens with tentative positive results in the Screening, additional hair is extracted from the initial sample and readied for Extraction. During this phase, drugs are isolated from the hair at significantly lower concentrations compared to other tests (such as urine or oral fluids), which contributes to the complexity of conducting hair drug screenings.

GC/MS, GC/MS/MS, or LC/MS/MS techniques perform the confirmation of positive screening results. Samples with tentative positives are cleaned prior to confirmation if necessary. The complete laboratory process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair programme and the accreditation according to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Long detection range: Hair drug tests can identify drug use up to 90 days back, unlike urine tests with a shorter detection range.
  • Resistant to tampering: Cheating on a hair drug test is almost impossible, enhancing result reliability.
  • Reveals usage patterns: It indicates patterns of drug use over time, rather than only recent usage.

Limitations:

  • Not suitable for recent use: Drugs take about 5-7 days to appear in the hair.
  • More costly: Hair drug tests generally cost more than other testing methods.
  • Result variability: Elements such as hair color and individual growth differences can impact metabolite concentration in the hair.

Note: Commonly termed as "hair follicle tests", the test analyzes the hair strand itself, not the follicle beneath the scalp.
