What it is, How it works

Hair testing has emerged as an effective tool for identifying drug and alcohol use. Hair captures a long-term record of alcohol and drug consumption by embedding biomarkers within the fibers of the hair strand as it grows. A sample obtained near the scalp can offer an approximate three-month detection period for alcohol and other substances. Hair is easy to gather, hard to tamper with, and simple to send.

A sample measuring 1.5 inches of around 200 hair strands near the scalp (approximately the diameter of a #2 pencil) equates to 100mg of hair, which is optimal for testing and validation. For EtG, additional tests, and/or those exceeding 10 panels, a 150mg specimen is advised. We suggest using a jeweler's scale for weighing the sample. If scalp hair cannot be collected, an equivalent quantity of body hair may be used. When mentioning head hair, we exclusively mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The lab procedures for processing a drug test result include four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary sample processing into the lab’s system. This step confirms the sample was properly sealed and delivered, assigns a random LAN (Laboratory Accessioning Number), and finishes any remaining data entry not completed by an electronic chain of custody system.

Screening involves an initial rapid drug abuse check. Although Screening is cost-effective for excluding drug use on most samples, a positive screen requires confirmation to be acceptable in court. Samples that are provisionally positive in Screening must undergo further verification.

If a sample tests provisionally positive during Screening, additional hair is taken from the initial sample for Extraction. In this phase, drugs are isolated from hair at significantly lower concentrations than other techniques (e.g., urine or oral fluids), enhancing the complexity of hair drug screening.

All screen-positive results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positives are washed pre-confirmation as necessary. From Accessioning through Confirmation, all laboratory steps are conducted under the CAP (College of American Pathologists) Hair designation and adhere to ISO / IEC 17025 standards.
