What it is, How it works

Hair testing is recognized as an effective method for identifying drug and alcohol consumption. The fibers of growing hair strands encapsulate biomarkers, offering a long-term record of alcohol and drug use. When taken from near the scalp, hair can provide a detection window of up to about three months for alcohol and drugs. It is easy to collect, relatively hard to tamper with, and simple to transport.

A 1.5-inch sample containing about 200 strands of hair (approximately the diameter of a #2 pencil) taken from the scalp will yield 100mg of hair, which is the ideal amount for analysis and confirmation. For EtG, add-ons, or tests surpassing 10 panels, a 150mg sample is advised. We recommend using a jeweler's scale to weigh the sample. If scalp hair is not available, an equivalent amount of body hair can be collected. When we mention head hair, we refer to scalp hair only. Body hair includes all other types of hair (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing a sample into a laboratory’s system. This step ensures the sample was sealed and transported correctly, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data entry that an electronic chain of custody system does not provide.

Screening is a quick preliminary test for drugs of abuse. Although Screening is an economical way to dismiss drug use in most samples, a Scanning positive test must be confirmed for it to be court-admissible. Any samples with presumptive positive results in Screening require further confirmation.

If a sample is presumptive positive during Screening, more hair is taken from the original sample and readied for Extraction. At this stage, drugs are extracted from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed as required before confirmation. The complete laboratory process from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
