When obtained close to the scalp, hair can present up to a 90-day detection period for alcohol and substances. Hair is straightforward to gather, relatively difficult to tamper with, and convenient to transport.

A sample length of 1.5 inches, constituting about 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal quantity for analysis and validation. For EtG, additional tests, and/or panels exceeding ten, 150mg of the sample is advised. We suggest weighing the sample using a jeweler’s scale. Should scalp hair be unavailable, an equivalent quantity of body hair can be collected. When addressing head hair, we specifically mean scalp hair. Body hair includes all other categories of hair (facial, axillary, etc.).

Process Overview

The primary steps in the laboratory handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the early processing of a specimen into a lab’s system. This involves confirming the sample was correctly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data input absent in an electronic chain of custody protocol.

Screening consists of an initial rapid assessment for substances of abuse. Although Screening is an economical approach to exclude drug usage for most samples, a positive finding requires verification to be valid in legal settings. Samples showing presumptive positivity in Screening necessitate further confirmation.

If a specimen shows presumptive positivity during the Screening phase, additional hair is extracted from the original sample and prepped for Extraction. Here, drugs are drawn from hair at a lower concentration than is typical in other methods (e.g., urine or oral fluid), which makes hair drug screening a challenging method to conduct.

Verification of any positive Screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples are cleaned before confirmation when necessary. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair standards and the accreditation per ISO / IEC 17025 benchmarks.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
