For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg sample is advisable. It’s recommended to measure the sample using a jeweler's scale. In cases where scalp hair is unavailable, an equivalent amount of body hair may be sourced. When discussing head hair, we refer solely to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a laboratory’s system. This process includes verifying the sample's proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not facilitated by an electronic chain of custody system.

During Screening, an initial rapid check for substances of abuse is conducted. Confirmation of positive screening results is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples undergo necessary washing. The complete laboratory procedure, from Accessioning to Confirmation, is subjected to review under both the CAP (College of American Pathologists) Hair designation and accreditation by ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
