What it is, How it works

Hair testing is now regarded as a significant means for identifying drug and alcohol consumption. By embedding biomarkers within its fibers, hair retains a long-term record of alcohol and drug usage. When gathered near the scalp, hair can offer up to a 3-month detection timeframe for alcohol and drugs. Hair is straightforward to gather, tough to tamper with, and simple to transport.

A 1.5-inch section of roughly 200 hairs (about the diameter of a #2 pencil) closest to the scalp results in 100mg of hair, which is the preferred sample size for screening and confirmation. For EtG, additional tests, and/or over 10-panel screens, a 150mg sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is not available, an equivalent portion of body hair may be used. References to head hair pertain solely to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four fundamental steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step involving the process of inputting a sample into the laboratory’s system. This step ensures the sample was properly sealed and transported, assigns a unique LAN (Laboratory Accessioning Number), and finalizes any additional data entry not captured by an electronic chain of custody system.

Screening involves a preliminary rapid test for drugs of abuse. While Screening serves as a cost-efficient method to dismiss drug usage in most samples, a positive Screening necessitates confirmation for legal admissibility. Samples that are possibly positive in Screening require a secondary Confirmation process.

In the case of a preliminary positive result from Screening, additional hair is extracted from the original sample for the Extraction phase. During this stage, drugs are separated from hair at a lower concentration compared to other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to perform.

Confirmation for any positive screening outcome utilizes GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positives are cleaned prior to Confirmation as necessary. The full lab procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
