What it is, How it works

Hair analysis is now recognized as an effective method for detecting substance use, providing a detailed history through biomarkers trapped in hair fibers. Collected near the scalp, hair can indicate alcohol and drug use over a period of up to 3 months. It is easy to acquire, hard to alter, and straightforward to transport.

A 1.5-inch cut of roughly 200 hair strands (approximately #2 pencil thickness) from the scalp yields 100mg, an ideal quantity for tests. For EtG, add-ons, and/or tests beyond 10 panels, 150mg is preferred. Using a jeweler’s scale is suggested for accuracy. If scalp hair isn’t accessible, a similar amount from body hair can be used. Head hair pertains only to scalp hair, while body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result encompasses Accessioning, Screening, Extraction, and Confirmation.

Accessioning initiates sample processing into the lab’s system. This involves checking sample sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and entering any data not already in an electronic chain of custody.

Screening is an initial quick test for drugs of abuse. While economical for ruling out usage in most samples, a positive screen requires confirmation for court admissibility. All samples presumptively positive in screening must be confirmed.

If presumption is positive during Screening, additional hair is gathered for Extraction. Here, drugs are extracted from hair at low concentrations, making hair drug screening a complex process.

Confirmation of positive screens is done using GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples are washed prior to confirmation as needed. The full lab process from Accessioning to Confirmation is under the purview of CAP (College of American Pathologists) and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
