What it is, How it works

Hair retains a prolonged account of alcohol and other drug consumption by embedding biomarkers within the hair strand's fibers as it grows. When taken from near the scalp, it can offer a detection range of up to roughly 3 months for both alcohol and drugs. Hair collection is straightforward, quite tamper-resistant, and easy to send for analysis.

A sample of 1.5 inches consisting of roughly 200 hair strands (comparable to a #2 pencil) nearest to the scalp yields 100mg, which is the optimal sample size for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, it is advised to use 150mg of the sample. It is suggested to measure the sample using a jeweler’s scale. If scalp hair is not available, a similar quantity of body hair can be used. Note that head hair refers exclusively to scalp hair, whereas body hair includes all other varieties (such as facial or axillary hair).

Process Overview

There are four core steps in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is first integrated into the laboratory’s system. This stage includes checking whether the sample was sealed and delivered correctly, assigning a random LAN (Laboratory Accessioning Number), and finishing any outstanding data entry not covered by an electronic chain of custody process.

Screening serves as an initial rapid assessment for drug abuse. Although it is a cost-efficient method to exclude drug use in most samples, any positive screening result must be verified for it to be valid in court. Samples testing presumptively positive in Screening require additional confirmation.

If Screening suggests a sample is positive, additional hair is extracted from the primary sample for the Extraction phase. Here, drug extraction occurs at a lower concentration level than other techniques (such as urine or oral fluid), making hair drug testing a challenging method to execute.

The Confirmation of any positive screen is performed utilizing GC/MS, GC/MS/MS, or LC/MS/MS techniques. Before Confirmation, any samples with a presumption of being positive are cleaned as necessary. The full lab procedure from Accessioning through Confirmation is assessed in accordance with both the CAP (College of American Pathologists) Hair standard and the accreditation under ISO / IEC 17025 norms.
