A 1.5-inch portion of around 200 hair strands (similar in size to a #2 pencil) near the scalp yields 100mg, which is the preferred sample size for both screening and confirmation. For EtG, additional testing, or tests exceeding 10 panels, a 150mg sample is advisable. We suggest using a jeweler’s scale for accurate measurement. In cases where scalp hair isn't accessible, a similar volume of body hair may be collected. "Head hair" refers to scalp hair specifically, whereas "body hair" encompasses all other hair types (e.g., facial, axillary).

Overview of the Process

The laboratory processing of a drug test result includes four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of incorporating a sample into the lab's system. This process verifies that the sample was correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and fills in any additional data not accounted for by an electronic chain of custody system.

Screening involves an initial, rapid assessment for substance abuse. Although Screening is a cost-efficient method to exclude drug use in most samples, a positive screening necessitates confirmation for legal acceptability. Samples that show an initial positive result in Screening require a follow-up confirmation.

If Screening results are presumptively positive, further hair is extracted from the original sample for the Extraction phase. Here, drugs are isolated from hair at significantly lower levels than in other methods (e.g., urine or oral fluid), underpinning why hair drug testing is the most intricate methodology.

Confirmation of positive Screening outcomes occurs via GC/MS, GC/MS/MS or LC/MS/MS. All samples that test presumptive positive are cleansed prior to confirmation as appropriate. The complete lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
