A 1.5-inch portion comprising roughly 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal quantity for both screening and confirmation tests. For EtG, extended panels, and tests exceeding 10 panels, a sample of 150mg is suggested. It is advisable to weigh the hair sample using a jeweler's scale. If scalp hair isn't available, a comparable amount of body hair can be utilized. By head hair, we specifically mean scalp hair, whereas body hair includes all other types (like facial, axillary, etc.).

Procedure Synopsis

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a specimen within a lab's system. This step ensures that the specimen was properly sealed and transported, assigns a unique LAN (Laboratory Accessioning Number), and completes additional data entry not covered by an electronic chain of custody system.

Screening offers a preliminary quick assessment for drug usage. Although Screening is a cost-efficient method to rule out drug intake in most samples, a positive screen must be confirmed for it to be legally valid. Any samples that yield a presumptive positive result in Screening require further confirmation.

Should a specimen yield a presumptive positive in the Screening phase, additional hair is extracted from the original sample and prepared for Extraction. Here, drugs are extracted from hair at significantly lower concentrations relative to other techniques (e.g., urine or oral fluid), which makes hair drug screening the most complex technique to execute.

Confirmation of any positive result from Screening is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens undergo washing prior to confirmation as necessary. The full laboratory process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
