What it is, How it works

Hair captures a prolonged history of substances by embedding biomarkers within the strands' fibers as they grow. When taken from the scalp's proximity, hair can indicate substance presence over about a three-month period. Hair sample collection is straightforward, is not easily falsified, and can be transported without hassle.

Approximately a 1.5-inch section comprising around 200 hair strands (similar to the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, which is perfect for analysis. For EtG tests, additional screenings, or panels exceeding ten, a specimen of 150mg is preferred. We suggest using a jeweler’s scale to weigh the hair. If scalp hair isn't available, an equivalent body hair amount can be used. "Head hair" refers strictly to scalp hair, while "Body hair" encompasses all other kinds (e.g., facial, axillary).

Process Overview

The laboratory processing of drug tests includes four chief steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning concerns the early stages of integrating a sample into the lab system. This entails ensuring the sample is properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and inputting any extra data not given by an electronic chain of custody.

Screening offers an initial assessment for drug use. Although Screening is economical for eliminating drug presence in most cases, a positive outcome mandates confirmation for legal admissibility. Samples that test positive initially in Screening must undergo a confirmation step.

Should a sample test positive in the Screening phase, additional hair is extracted and prepared for the Extraction phase, during which drugs are isolated from hair at significantly reduced concentrations versus other methods (e.g., urine, oral fluid), hence hair testing's status as the most intricate approach.

Final Confirmation for any positive findings is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Pre-confirmation washing is standard for all samples with presumptive positives. The complete lab sequence, from Accessioning through Confirmation, undergoes scrutiny under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
