What it is, How it works

Hair evaluation is now recognized as an effective method for identifying drug and alcohol consumption. Hair strands capture biomarkers as they grow, providing a long-term record of alcohol and drug use. When taken from near the scalp, hair can offer up to about a 3-month period for detecting alcohol and other substances. Hair collection is straightforward, somewhat resistant to tampering, and easy to mail.

A 1.5-inch section consisting of roughly 200 hair strands (similar in size to a #2 pencil) closest to the scalp offers 100mg of hair, which is the perfect amount for both initial screening and follow-up testing. For EtG enhancements and/or assessments beyond the 10-panel range, a 150mg sample is advised. It’s recommended to measure the collected sample with a jeweler’s scale. If scalp hair isn't available, an equal quantity of body hair can be gathered. References to head hair pertain only to scalp hair. Body hair includes all other types (like facial and axillary hair).

Process Overview

The process of laboratory handling of a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into the lab’s system. This phase ensures the sample was properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entries not covered by an electronic custody tracking system.

Screening consists of a preliminary quick test for substances. Although Screening is an economical method for excluding drug use in most samples, positive results require confirmation for legal acceptance. Samples with initial positive results in Screening need an additional confirmation step.

If the Screening phase results in a presumptive positive, further hair is taken from the original sample and readied for Extraction. In this phase, drugs are drawn from hair at a lower concentration compared to other testing methods (such as urine or oral fluid), which is why hair drug screening is one of the more challenging procedures to conduct.

Confirmation of any positive Screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation when necessary. The full laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
