For EtG add-ons or tests over 10 panels, it is advisable to use 150mg of the sample. A jeweler’s scale is suggested for weighing the specimen. If scalp hair cannot be collected, a comparable quantity of body hair is acceptable. "Head hair" is used specifically for scalp hair, whereas "body hair" pertains to all other types (face, armpits, etc.).

Process Overview

There are four primary stages in laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s database, which involves confirming the sample’s proper seal and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not included in an electronic chain of custody system.

Screening provides a quick preliminary assessment for drug presence. Although cost-efficient, a positive result must be confirmed for legal admissibility. Any samples with presumed positives in Screening need further confirmation.

Upon obtaining a presumptive positive result during Screening, additional hair is isolated from the original sample and readied for Extraction. During this phase, drugs are extracted from the hair at much lower concentrations than in other methods (e.g., urine or oral fluid), underscoring why hair drug screening presents the greatest challenge methodologically.

Confirmation of any positive screen occurs via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumed positives are washed as necessary. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
