A sample measuring 1.5 inches, consisting of roughly 200 hairs (about the size of a #2 pencil) adjacent to the scalp, yields 100mg of hair, which is an optimal amount for examination and verification. For EtG, additional tests, and panels exceeding 10, a 150mg specimen is advised. We suggest using a jeweler’s scale to measure the specimen weight. If scalp hair is not available, a comparable quantity of body hair is an alternative. References to head hair pertain to scalp hair exclusively. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves incorporating the sample into a laboratory’s system. This encompasses checking that the sample is sealed and shipped correctly, assigning a unique LAN (Laboratory Accessioning Number), and performing any extra data input not covered by an electronic chain of custody.

Screening is a preliminary rapid assessment for abused substances. While Screening is cost-effective for excluding drug use in the majority of specimens, a positive screen must be verified for legal acceptability. Any samples indicating potential positive results during Screening must undergo further confirmation.

Should a sample appear positive during the Screening phase, additional hair is taken from the original sample for Extraction. This stage involves extracting drugs from hair at a lower concentration than other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to conduct.

Confirmation of any screening positivity is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples are cleansed before confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is evaluated under CAP (College of American Pathologists) standards for Hair and ISO / IEC 17025 certification.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Furthermore, the SoHT meeting in Verona featured research on the identification and quantification of alcohol abuse biomarkers, their stability, and their correlation with exposure and other biomarkers.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
