A sample measuring 1.5 inches consisting of approximately 200 hair strands (equivalent to a #2 pencil) near the scalp yields 100mg of hair, considered the optimal quantity for analysis. For tests including EtG, additional panels, or those exceeding 10, 150mg of the sample is suggested. Using a jeweler’s scale is advised for measuring the sample weight. If there's an absence of scalp hair, a comparable quantity of body hair may be used. References to head hair pertain solely to scalp hair, while body hair encompasses all other types (like facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial entry into the lab’s system. This involves confirming proper sealing and shipping of the sample, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody record.

Screening provides a preliminary assessment for drug substances. Although Screening is a cost-efficient method to exclude drug use for most samples, a confirmed result is necessary for judicial approval. Samples with a preliminary positive result during Screening need secondary confirmation.

For samples with a presumptive positive in the Screening step, further hair is extracted from the primary sample for the Extraction process. Drugs are isolated from hair at a more diluted concentration compared to other methods (such as urine or oral fluid), making hair drug analysis the most complex method to execute.

Confirmation of any positive screen results is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation when necessary. The complete process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation along with accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
