For EtG, extended panels, and/or analyses exceeding 10 panels, it is advisable to have 150mg of the sample. We advise using a jeweler’s scale for weighing purposes. In instances where head hair is unavailable, a comparable quantity of body hair can be utilized. References to head hair specifically indicate scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Procedure Outline

The pivotal stages in laboratory drug test processing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial intake of a sample into the lab's system. This involves ensuring the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any outstanding data entry absent from the electronic chain of custody input.

Screening comprises a preliminary rapid assessment for illicit drug presence. Although Screening is economically advantageous in precluding drug use in most samples, a positive screen necessitates verification to be legally valid. Samples that render a presumptive positive in Screening require further validation.

Should a sample manifest as presumptively positive at the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. At this point, drugs are isolated from hair at concentrations significantly lower than other methods (e.g., urine or oral fluid), thereby making hair drug screening a complex methodology.

Verification of positive screen outcomes is conducted via GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as appropriate. The comprehensive lab process spanning from Accessioning to Confirmation undergoes examination compliant with both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
