What it is, How it works

Hair retains a record of alcohol and drug consumption by capturing biomarkers within the strands as they grow. When hair from close to the scalp is used, it offers up to a 3-month timeline for detecting alcohol and other drugs. Hair collection is straightforward, resistant to tampering, and easy to transport.

A 1.5-inch section comprising about 200 hair strands (approximately the width of a #2 pencil) from near the scalp provides 100mg of hair, which is optimal for initial screening and confirmation. For EtG, enhancements, and/or tests exceeding 10 panels, 150mg of sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair cannot be obtained, an equivalent amount of body hair is acceptable. Here, head hair means scalp hair specifically. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The key phases in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a sample into the lab’s system. This step verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry absent from an electronic chain of custody system.

Screening offers an initial rapid assessment for substances of abuse. Although Screening is an economical means to exclude drug usage in most samples, a positive screen must be confirmed to be court-admissible. Samples flagged as presumptively positive during Screening need further confirmation.

Upon a presumptively positive result in Screening, additional hair is extracted from the original sample for the Extraction phase. Here, drugs are extracted from hair at considerably lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting the complexity of conducting hair drug screening.

Confirmation for any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing pre-confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.
