For EtG, additional components, and tests involving more than 10 panels, a specimen of 150mg is advisable. Utilizing a jeweler’s scale for precise specimen weight is recommended. In instances where scalp hair cannot be collected, a similar quantity of body hair may suffice. References to head hair pertain solely to scalp hair, while body hair encompasses all other types, including facial and axillary hair.

Process Overview

The laboratory processing of a drug test consists of four essential phases: Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, a sample transitions into a lab’s system, involving confirmation of the sample’s proper sealing and shipment, allocation of a random LAN (Laboratory Accessioning Number), and completion of any missing data entry absent from an electronic chain of custody system.

Screening performs an initial expedited evaluation for potential substance abuse. Though it offers a budget-conscious means of eliminating drug use suspicions for most samples, a positive result during Screening necessitates confirmation to be legally admissible. Any samples yielding positive presumptive results during Screening require additional verification.

When a sample shows a presumptive positive result in Screening, more hair is extracted from the initial collection and prepared for the Extraction phase. During this phase, substances are separated from hair in significantly smaller concentrations than in methods such as urine or oral fluids, underscoring the complexity of conducting hair drug screening.

Positive Screening outcomes are verified using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing prior to confirmation when necessary. The entire lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and abides by the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
