For EtG tests, add-on tests, and/or tests exceeding 10 panels, a 150mg specimen is advised. Measuring the specimen on a jeweler’s scale is recommended. If scalp hair is unavailable, an equivalent quantity of body hair can be collected. Head hair refers exclusively to scalp hair, while body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample undergoes initial processing into a lab’s system. This process involves verifying the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening is a preliminary check for substances of abuse. Though Screening economically rules out drug use in the majority of cases, any positive screen requires confirmation to be legally recognized. Samples showing positive results in Screening necessitate secondary confirmation.

If included in the Screening stage, more hair is taken from the initial sample for Extraction. In this step, drugs are extracted from hair at significantly lower concentrations compared to other methodologies (e.g., urine or saliva), hence making hair drug screening the most challenging method to perform.

Confirmation of any positive Screening outcome is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all suspected positive samples undergo washing as necessary. The complete lab procedure from Accessioning to Confirmation is inspected under both the CAP (College of American Pathologists) Hair designation and the accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
