Collecting a 1.5-inch segment of about 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal quantity for screening and verification. For EtG, add-ons, and/or tests beyond 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale for specimen weighing. If scalp hair is not available, a similar quantity of body hair can be gathered. Reference to head hair pertains exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four principal stages in laboratory assessment of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial handling of the sample into the lab's framework. This process involves verifying the integrity and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any manual data entries not facilitated by an electronic chain of custody system.

Screening involves an initial rapid assessment for substances of misuse. Although Screening is cost-efficient for excluding drug use in most samples, a positive screen necessitates confirmation for legal admissibility. Samples that are tentatively positive in Screening require secondary confirmation.

If Screening indicates a tentative positive, additional hair is extracted from the original sample and conditioned for Extraction. In this stage, drugs are extracted from hair at much lower concentrations than other methods (such as urine or oral fluid), contributing to the complexity of hair drug screening processes.

Confirmation of any tentative positive screening outcome is executed via GC/MS, GC/MS/MS or LC/MS/MS. All tentatively positive samples undergo washing before confirmation when necessary. The comprehensive laboratory sequence from Accessioning to Confirmation is vetted under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
