Process Overview

The four key steps in laboratory processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing a sample within a lab's system. This step verifies the sample was appropriately sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary data entry missing from an electronic chain of custody system.

Screening is an initial rapid assessment for drugs of abuse. Though cost-effective for ruling out drug usage in most samples, a positive screen must be verified for legal admissibility. Samples with presumptive positive results require a secondary confirmation.

If a sample tests presumptively positive at Screening, additional hair from the primary sample is readied for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most complex methodology to execute.

Confirmation of any positive screening outcome uses GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptively positive samples are washed before confirmation if necessary. The entire laboratory process—from Accessioning to Confirmation—is reviewed under the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.
