A 1.5-inch section comprising about 200 hair strands (equivalent to a #2 pencil) nearest to the scalp produces 100mg of hair, optimal for screening and confirmation. For EtG extensions and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the sample with a jeweler’s scale. Should scalp hair be unavailable, an equivalent quantity of body hair can be gathered. Mention of head hair pertains solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result includes four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is integrated into a laboratory system. This process includes affirming the proper sealing and shipment of the sample, allocating a random LAN (Laboratory Accessioning Number), and finalizing any necessary data entry absent from an electronic chain of custody system.

Screening serves as a preliminary swift assessment for substance abuse. While Screening offers an economical means to negate drug use for most samples, a positive result requires confirmation for court admissibility. Samples that test presumptively positive during Screening necessitate further validation.

Should Screening produce a presumptively positive sample, additional hair is extracted from the initial sample for the Extraction phase. In this phase, drugs are drawn out from hair at much lower concentrations compared to alternative methods (e.g., urine or oral fluid), enhancing the complexity of performing hair drug screening.

Confirmation for any positive screening outcome utilizes GC/MS, GC/MS/MS, or LC/MS/MS technologies. Preceding confirmation, presumptive positive samples undergo necessary washing. The comprehensive laboratory progression from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
