A 1.5-inch sample consisting of approximately 200 hair strands (roughly the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the perfect sample size for both screening and confirmation. For EtG, additional tests, and/or analyses exceeding 10 panels, 150mg of the specimen is advised. We suggest weighing the specimen on a jeweler’s scale. Should scalp hair be absent, an equivalent measure of body hair may be used. When referencing head hair, it only pertains to scalp hair. Body hair indicates all other hair (facial, axillary, etc.).

Process Overview

The principal procedures in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample within a lab's system. This phase confirms that the sample was properly sealed and shipped, assigns an arbitrary LAN (Laboratory Accessioning Number), and completes any additional data input not covered by an electronic chain of custody system.

Screening consists of a preliminary rapid evaluation for substance misuse. Although Screening is a cost-effective method for dismissing drug consumption in most cases, any positive result must be confirmed to be valid in legal contexts. All samples that show a presumptive positive in Screening must undergo secondary confirmation.

Should a sample display a presumptive positive during Screening, additional hair is extracted from the original sample and prepared for Extraction. During Extraction, drugs are withdrawn from hair in much lower concentrations than other techniques (e.g., urine or oral fluid), making hair drug analysis the most complex technique to execute.

Confirmation of any positive Screening result is performed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing before confirmation, as necessary. The complete laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
