When clipped close to the scalp, hair may offer up to a three-month window for detecting alcohol and other substances. The collection process for hair is straightforward, it is relatively resistant to tampering, and it is easy to transport.

A 1.5-inch segment comprising around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is optimal for both screening and verification. For EtG supplements and/or assessments beyond 10 panels, a sample size of 150mg is advised. We suggest measuring the sample with a jeweler’s scale. In the absence of scalp hair, an equivalent volume of body hair can be taken. In discussing head hair, we mean scalp hair specifically. Body hair encompasses all other hair varieties (such as facial or axillary hair).

Process Overview

The four primary stages in the lab handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial setup of a sample within a lab's system. This process includes confirming the correct sealing and shipping of the sample, allocating a random LAN (Laboratory Accessioning Number), and finishing any extra data input not furnished by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. Although screening is an economical method to eliminate drug usage from the majority of samples, any positive findings need further verification to be court-admissible. Samples giving a presumptive positive in screening require an additional confirmation step.

Upon a presumptive positive outcome in screening, additional hair is extracted from the initial sample for preparation in the Extraction phase. Here, substances are removed from hair at substantially lower concentrations than alternative methods (such as urine or oral fluid), which makes hair drug testing one of the most complicated methodologies to execute.

Positive screening results in confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, necessary washing is done on all presumptive positive samples. The complete lab process, from Accessioning to Confirmation, is evaluated under the CAP (College of American Pathologists) Hair standards and aligns with the ISO / IEC 17025 accreditation.
