A sample size of approximately 200 hair strands (similar in diameter to a #2 pencil) about 1.5 inches long from the scalp offers 100mg, the optimal amount for both screening and confirmation. For beyond 10 panels, EtG, or additional tests, a 150mg sample is advised. Employing a jeweler’s scale is suggested for weighing. If scalp hair isn’t an option, an equivalent quantity of body hair can be used. References to head hair solely mean scalp hair, while body hair includes any other types (facial, etc.).

Process Summary

Laboratory processing of drug test results involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes integrating a sample into a lab’s system by confirming that it was sealed and shipped correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any missing data through an electronic chain of custody.

Screening provides an initial rapid assessment for drug usage. Though Screening efficiently eliminates negative samples, a positive result must be validated for potential court use. Samples that appear positive in Screening necessitate secondary confirmation.

For samples testing positive in Screening, additional hair from the original sample undergoes Extraction. This process extracts drugs from hair at concentrations lower than other methods (like urine or oral fluid), making hair drug testing a challenging technique.

Confirmation of any positive Screening is done using GC/MS, GC/MS/MS, or LC/MS/MS. Samples indicated as positive are washed if required prior to confirmation. The complete laboratory process, from Accessioning to Confirmation, is conducted following CAP (College of American Pathologists) Hair guidelines and ISO/IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
