Hair is straightforward to collect, relatively challenging to tamper with, and convenient to transport.

A 1.5-inch segment of around 200 strands of hair (approximately the width of a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the optimal quantity for analysis and validation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is recommended. We advise measuring the sample on a jeweler’s scale. If scalp hair is unobtainable, an equivalent quantity of body hair can be collected. When discussing head hair, we mean scalp hair solely. Body hair alludes to all other forms (facial, axillary, etc.).

Process Overview

The principal stages in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the primary processing of a specimen into a laboratory's system. This involves corroborating that the specimen was sealed and sent correctly, assigning a non-sequential LAN (Laboratory Accessioning Number), and completing any extra data entry not encapsulated by an electronic chain of custody system.

Screening represents an initial rapid assessment for drugs of misuse. While Screening is an economical approach to disregard drug use in most samples, a positive screen is mandatory to be verified to be valid in court. Any specimen that registers positive in Screening necessitates further confirmation.

If a specimen shows a positive indication in the Screening phase, additional hair is extracted from the original sample and prepped for Extraction. In this phase, drugs are isolated from hair at a substantially lesser concentration compared to other techniques (e.g., urine or oral fluid), which explains why hair drug screening is the most challenging technique to perform.

Confirmation of a positive screening outcome is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicative of positive are cleansed before confirmation as deemed necessary. The entire lab procedure from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair designation and is accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
